Information we collect:
- Contact Information: Your name, phone number, email address, and any details regarding your bookings.
- Appointment and Referral Information: The type of exam requested, preferred times, and requisition documents you choose to upload.
Records, diagnostic images, and reports are handled under PHIPA and are kept safe and secure. Your privacy is our priority.
We are committed to ensure that all patient complaints are dealt with thoroughly and promptly.
If you have a complaint concerning the care you received or in regards to the operation of our clinic please feel free to contact our Management Team by email at info@xus.ca , or feel free to give your details to one of our staff members who will pass on your information to the Management Team.
All written complaints will be investigated and a response will be provided back to you within 10 business days.
If for some reason you are not happy with the resolution of the complaint, or if you feel we have not dealt with the complaint in a timely fashion, it is your right to bring your complaint to the Patient Ombudsman under the Excellent Care for All Act, 2010.
